Obesity doesn’t affect everyone in the same way.
Two people may have a similar Body Mass Index (BMI), yet their day-to-day lives can look completely different. One person may be managing Type 2 diabetes, sleep apnoea and high blood pressure, while another struggles most with mobility, joint pain or chronic fatigue. Some people have lived with obesity since childhood, while others have seen their weight increase steadily over the years.
This is why treating severe obesity is never as simple as choosing a bariatric procedure.
When your BMI is higher, your treatment needs to be planned around your health, your medical history and the challenges you’re facing—not just the number on the scales.
At Blackrock WeightCare, every patient receives an individual assessment because the right treatment is different for every person.
A Higher BMI Brings Different Challenges
Living with severe obesity can affect almost every aspect of daily life.
Simple activities that many people take for granted can become increasingly difficult. Walking around the supermarket, climbing stairs, getting in and out of a car or enjoying a family holiday may require far more effort than they once did.
As BMI increases, the likelihood of obesity-related conditions also rises. These may include:
- Type 2 diabetes
- Obstructive sleep apnoea
- High blood pressure
- Fatty liver disease
- Joint pain and reduced mobility
- Heart disease
These health conditions often become more difficult to manage over time, making early specialist assessment increasingly important.
Bariatric Surgery Is Never About Choosing the “Best” Procedure
One of the biggest misconceptions about bariatric surgery is that there is one procedure that suits everyone.
There isn’t.
A gastric sleeve may be an excellent option for one patient, while another may benefit more from a gastric bypass or SADI-S. The decision depends on several clinical factors rather than BMI alone.
During your consultation, your surgeon considers much more than your weight.
Your assessment includes:
- Your overall health
- Existing obesity-related conditions
- Previous abdominal or bariatric surgery
- Eating habits
- Nutritional health
- Current medications
Every recommendation is made with one goal in mind – to identify the treatment that offers the safest and most effective long-term outcome for you.
Why Patients With a Higher BMI Often Need a Different Approach
Severe obesity presents unique challenges before, during and after surgery.
Patients with a higher BMI often require more detailed preparation before an operation. This may involve improving nutritional health, managing existing medical conditions, following a liver reduction diet and ensuring every aspect of treatment has been carefully planned.
The operation itself also requires experience.
From anaesthetic planning to the surgical technique and post-operative recovery, every stage of care should be designed around the individual rather than following a standard pathway.
This level of planning is one of the reasons specialist bariatric centres play such an important role in treating complex obesity.
Why SADI-S May Be Recommended for Some Patients
Not every patient with a high BMI requires the same bariatric procedure.
For some people, a gastric sleeve provides an excellent outcome. Others may achieve better long-term results with a gastric bypass.
There are also patients for whom Single Anastomosis Duodeno-Ileal Bypass with Sleeve Gastrectomy (SADI-S) may be the most appropriate option.
SADI-S combines a sleeve gastrectomy with a bypass of part of the small intestine. The procedure is designed to reduce food intake while also producing powerful metabolic changes that can support significant and sustained weight loss and improvements in obesity-related conditions such as Type 2 diabetes.
Like every bariatric procedure, SADI-S is not suitable for everyone. It requires lifelong vitamin supplementation, nutritional monitoring and regular follow-up.
This is why it should only be recommended after a comprehensive assessment by an experienced multidisciplinary bariatric team.
Experience Makes a Difference
High BMI patients often require a different level of assessment and surgical planning. Decisions about the most appropriate procedure are influenced by much more than BMI alone and should take into account each patient’s overall health and obesity-related conditions.
Having dedicated his practice to bariatric surgery for many years, Mr William Robb has extensive experience in treating patients with severe and complex obesity. That experience, together with the support of Blackrock WeightCare’s multidisciplinary team, helps ensure every patient receives a personalised treatment plan based on their individual needs.
Success Isn’t Defined by a Number
People often begin their bariatric journey thinking only about weight loss.
Over time, their priorities usually change.
The conversations during follow-up appointments are rarely about reaching a particular number on the scales.
Instead, patients talk about coming off CPAP treatment, reducing diabetes medication, walking further without stopping, travelling comfortably, fitting into an aeroplane seat, returning to work or simply having the energy to enjoy time with family.
These are the milestones that matter.
They reflect improvements in health, independence and quality of life rather than a specific weight.
The Right Plan Starts With the Right Assessment
Choosing a bariatric procedure should never be based on BMI alone.
Every patient deserves a thorough assessment that looks beyond the scales and considers the complete picture.
At Blackrock WeightCare, your treatment plan is tailored to your individual needs. Our multidisciplinary team carefully evaluates your health, obesity-related conditions, previous treatments and long-term goals before recommending the approach that is most appropriate for you.
For some patients, that may be a gastric sleeve.
For others, it may be a gastric bypass or SADI-S.
The important thing is that the decision is based on clinical expertise and what will provide the safest and most effective outcome for your individual circumstances.
