Patients with a BMI over 50 rarely present with a single diagnosis. More often, they are living with type 2 diabetes, hypertension, obstructive sleep apnoea, osteoarthritis, fatty liver disease and significant mobility limitation at the same time. For many GPs across Ireland, this group represents some of the most complex cases in everyday practice.
At this level, obesity is not simply excess weight. It is a chronic, progressive disease affecting multiple organ systems simultaneously. The metabolic load is significant, and standard lifestyle advice alone is rarely enough to reverse the trajectory.
How comorbidities compound each other
These conditions do not exist in isolation. They amplify one another. Poor glycaemic control increases cardiovascular risk. Sleep apnoea drives resistant hypertension. Joint pain reduces mobility, which in turn promotes further weight gain. Fatty liver disease progresses silently in the background.
Over time, polypharmacy becomes routine. Despite appropriate prescribing and careful follow-up, disease burden often continues to increase. Patients can feel trapped in a cycle where each new medication addresses a symptom but not the underlying driver.
When medical management reaches its limits
There are often clear indicators that conventional management is no longer enough:
- Escalating insulin or multiple diabetic agents
- Increasing antihypertensive requirements
- Recurrent hospital admissions
- Progressive loss of mobility and independence
At this point, the discussion may need to shift from controlling complications to modifying the disease process itself.
