And one of the first things that surprises them is the answer: if you have private health insurance in Ireland and you meet the clinical criteria, there is a strong chance your surgery is already covered.
This is, as one of our own blog posts has noted, something of a hidden secret. Many patients assume bariatric surgery is an out-of-pocket expense — a luxury or an elective procedure. In reality, it is a clinically indicated medical treatment for a serious chronic condition, and Ireland’s three main insurers — VHI, Laya Healthcare, and Irish Life Health — all include bariatric surgery under qualifying plans at approved hospitals including Blackrock Clinic.

What Are the Eligibility Criteria?
Irish health insurers generally follow internationally recognised clinical guidelines when assessing eligibility for bariatric surgery. While specific requirements vary by insurer and plan, the typical criteria are:
Standard Clinical Eligibility Criteria
- BMI of 40 or above— regardless of other health conditions
- BMI of 35 or abovewith at least one significant obesity-related comorbidity such as type 2 diabetes, hypertension, sleep apnoea, or cardiovascular disease
- Evidence of previous attemptsat non-surgical weight management
- No contraindicationsto bariatric surgery — assessed through pre-operative evaluation
- Psychological suitability— assessed by a psychologist as part of the pre-operative programme
- Age typically18–65 years, though assessment is made case by case
Insurers may also require a minimum waiting period on the policy before a claim for planned surgical procedures can be made. This is known as a waiting period and typically applies to new policies. If you are on an existing policy and have not previously made a claim for bariatric surgery, this waiting period may already have passed.
Which Procedures Are Covered?
The most commonly covered procedures at Blackrock WeightCare include:
Gastric Sleeve (Sleeve Gastrectomy)
Covered under qualifying plans with all three major insurers. The most commonly performed bariatric procedure in Ireland, accounting for the majority of weight loss operations.
Gastric Bypass (Roux-en-Y)
Covered under qualifying plans. Particularly relevant for patients with type 2 diabetes, where bypass has the strongest evidence for long-term remission.
SADI-S (Single Anastomosis Duodeno-Ileal Bypass with Sleeve)
A more complex procedure for patients with higher BMI or where maximum metabolic benefit is required. Cover is plan-specific — our team will verify.
For patients who have had a previous bariatric procedure and require revision. Cover varies more significantly — we will assess your specific situation.
What If My Policy Doesn’t Cover Surgery?
If your current plan does not include bariatric surgery, there are options. Upgrading your policy to a higher-tier plan may include surgical cover — though waiting periods may apply to the upgraded benefits. It is also worth checking whether your corporate or employer-provided health insurance includes bariatric surgery. In recent years, Ireland’s major insurers have expanded their cover for corporate clients, and many employees are unaware that their group scheme includes this benefit. Your HR department or insurance provider can confirm what your corporate plan includes.
