Gastric bypass surgery can lead to significant weight loss and improvements in obesity-related health conditions. However, not everyone achieves the same results. Some patients may lose less weight than expected, while others may experience weight regain several years after their original surgery.

This can be particularly difficult for patients who have made considerable changes to their eating habits and lifestyle but are still living with severe obesity.

Needing further treatment does not mean that the patient or their original surgery has failed. Obesity is a chronic, complex disease, and its management may need to change over time.

The American Society for Metabolic and Bariatric Surgery (ASMBS) recognises that further treatment may be appropriate when patients experience insufficient weight loss or weight recurrence following bariatric surgery.

For some patients, revisional bariatric surgery may be an option. One procedure that may be considered in carefully selected cases is Single Anastomosis Duodeno-Ileal Bypass with Sleeve Gastrectomy (SADI-S).

However, revising a previous gastric bypass to SADI-S is a complex operation and is not suitable for everyone.

Why Might Someone Need Further Surgery After Gastric Bypass?

Gastric bypass works by changing the size of the stomach and the way food passes through the digestive system. It also influences gut hormones involved in appetite, fullness and blood glucose regulation.

While many patients maintain meaningful weight loss over the long term, some may require further treatment.

There are several reasons why revisional surgery might be discussed.

Insufficient Weight Loss

Some patients lose a considerable amount of weight following gastric bypass but remain within a BMI range associated with severe obesity.

For example, someone who started with a particularly high BMI may have achieved substantial weight loss but still have obesity-related health concerns.

In these circumstances, further treatment may be considered to help address the remaining clinical risks.

Significant Weight Regain

Some weight regain can occur in the years following gastric bypass.

Changes in appetite regulation, metabolism, eating patterns, medication and the anatomy of the original bypass may all contribute.

Weight regain does not automatically mean that another operation is needed. A careful assessment is important to understand what has changed and whether medical, nutritional or surgical treatment would be most appropriate.

As outlined in the ASMBS statement on treatment options after bariatric surgery, treatment may include medication, endoscopic procedures or revisional surgery, depending on the patient’s circumstances.

Ongoing Obesity-Related Health Conditions

Weight loss is not the only measure of success following bariatric surgery.

Some patients may continue to experience conditions such as:

Type 2 diabetes

High blood pressure

Obstructive sleep apnoea

Joint pain and reduced mobility

Other complications associated with severe obesity

Where significant obesity remains alongside these conditions, further treatment may be considered as part of an overall plan to improve long-term health.

What Is SADI-S and How Is It Different From Gastric Bypass?

SADI-S is a bariatric and metabolic procedure that combines a sleeve-shaped stomach with a bypass of part of the small intestine.

It works through changes in food intake, nutrient absorption and the hormonal signals involved in appetite and metabolism.

SADI-S is generally considered a more malabsorptive procedure than a standard Roux-en-Y gastric bypass, although the effects depend on the individual anatomy and surgical technique.

When a patient has already undergone gastric bypass, the operation is considerably more complex than performing SADI-S as a first bariatric procedure.

The surgeon must carefully assess the existing stomach and intestinal anatomy before determining whether reconstruction to a SADI-S configuration is technically possible and clinically appropriate.

Who Might Be Considered for Gastric Bypass to SADI-S Revisional Surgery?

There is no single BMI or weight regain threshold that automatically makes someone suitable for this type of revision.

However, certain circumstances may lead an experienced bariatric surgeon to consider whether it is an appropriate option.

Patients With Persistent Severe Obesity

Patients who continue to have a high BMI despite meaningful weight loss after gastric bypass may be considered for further assessment.

This may be particularly relevant where the remaining excess weight continues to affect mobility, metabolic health or quality of life.

The decision is based on more than BMI alone. The patient’s overall health, previous weight loss, surgical history and likely benefits and risks must all be considered.

Patients With Significant Weight Regain

For patients who have regained a substantial amount of weight, the first step is to understand why.

This may involve reviewing the original operation, examining the current anatomy and assessing nutritional, behavioural and medical factors.

If significant obesity persists and other treatment approaches have not provided sufficient benefit, revisional surgery may be discussed.

Patients Who Are Medically and Nutritionally Suitable

Revisional surgery requires careful consideration of the patient’s nutritional health.

Gastric bypass already affects the absorption of certain nutrients. SADI-S can increase the risk of protein and micronutrient deficiencies, making nutritional assessment particularly important.

The ASMBS nutritional guidelines emphasise the importance of screening for nutritional deficiencies and maintaining appropriate supplementation and monitoring following bariatric surgery.

Patients need to be able to follow long-term dietary guidance, take prescribed supplements and attend regular follow-up appointments.

Existing deficiencies may need to be corrected before surgery can be considered.

Why Is Assessment Particularly Important Before Revisional Surgery?

Revisional bariatric surgery is generally more technically demanding than a first bariatric operation.

Scar tissue, altered anatomy and changes from the previous procedure can increase surgical complexity.

An international consensus on revisional bariatric surgery highlights the importance of careful patient selection, multidisciplinary assessment and appropriate investigations before further surgery.

Before recommending further surgery, the multidisciplinary team may arrange investigations such as an upper gastrointestinal endoscopy, imaging and blood tests.

The assessment will also consider:

The type of gastric bypass originally performed

Weight loss and weight regain history

Current BMI and obesity-related health conditions

Existing nutritional deficiencies

Digestive symptoms and previous surgical complications

The patient’s ability to maintain long-term follow-up

These findings help determine whether further surgery is appropriate and, if so, which procedure may offer the most suitable outcome.

What About Recovery and Long-Term Care?

Recovery following revisional surgery varies depending on the complexity of the operation and the patient’s overall health.

As with other bariatric procedures, patients follow a staged dietary progression after surgery. However, the nutritional requirements following SADI-S need particular attention.

Long-term care includes monitoring protein intake, vitamin and mineral levels, weight changes and general health.

At Blackrock WeightCare, we recognise the importance of continued support following surgery. You can read more about our approach to weight loss aftercare.

Considering Further Surgery After Gastric Bypass?

Having undergone gastric bypass does not necessarily mean that all surgical treatment options have been exhausted.

For patients who continue to live with severe obesity or have experienced significant weight regain, a specialist assessment can help establish whether further treatment may be beneficial.

At Blackrock WeightCare, Mr William Robb and our experienced multidisciplinary team assess patients considering revisional bariatric surgery, including complex cases where SADI-S may be an option.

Every patient’s circumstances are different, and any recommendation for further surgery follows a careful review of their previous operation, current health and long-term needs.

One thing patients often tell me in the early weeks following bariatric surgery is, “I didn’t realise drinking enough would take this much thought.”

It is understandable. Most people preparing for weight loss surgery are thinking about how much they will be able to eat, what foods they can have and how quickly they might lose weight. Drinking water does not tend to be high on the list of concerns.

Yet hydration is something we pay close attention to during recovery.

Having cared for bariatric patients for many years, I have seen that the difficulty is rarely that someone does not understand the importance of drinking. More often, they are simply adjusting to the fact that they cannot drink in the same way they did before.

The good news is that, with a few simple habits, it usually becomes much easier.

Why can something as simple as drinking suddenly feel different?

Following a gastric sleeve, gastric bypass or SADI-S, taking in a large amount of fluid quickly may no longer feel comfortable, particularly during the early stages of recovery.

Some patients also experience nausea or changes in appetite and thirst. Add in everything else you are learning about your postoperative diet, protein, medications and activity, and it is easy to fall behind with fluids without noticing.

Dehydration is something we take seriously in bariatric care. Research involving more than 250,000 patients undergoing sleeve gastrectomy or gastric bypass found that dehydration requiring treatment was associated with emergency department visits and hospital readmission.

That does not mean hydration needs to become a source of worry. It means we want patients to understand it and establish good habits early.

Forget the large glass. Think little and often

Patients sometimes tell me, “I just can’t get through that much water.”

My advice is not to look at the whole day’s fluid requirement as one big task.

You are not trying to drink a large glass at once. Small amounts, taken regularly across the day, are generally much easier to manage.

Your bariatric team will advise you on your individual fluid requirements. International bariatric guidance commonly refers to approximately 64 fluid ounces, or around 1.9 litres, per day, although individual requirements and postoperative plans can vary.

A few practical habits can make a real difference:

Start your fluids early rather than trying to catch up later in the day.

Keep your drink somewhere visible and within reach.

Use a bottle with a known volume if that helps you keep track.

Sip regularly rather than taking large drinks.

Follow the guidance from your bariatric team about suitable fluids at each stage of recovery.

If you find plain water difficult to tolerate, let your bariatric team or dietitian know. There may be suitable alternatives depending on where you are in your postoperative plan.

You will need to rethink drinking with meals too

For many people, having a drink beside their meal is something they have done all their lives.

Following bariatric surgery, that habit needs to change.

BOMSS guidance recommends avoiding drinking with meals and leaving approximately 30 minutes between eating and drinking.

Initially, patients can find this one of the more awkward adjustments.

I usually reassure them that it becomes much more natural with time. What feels like a schedule during the first few weeks eventually becomes part of your normal routine.

Your body will usually give you a few clues

You do not need to become obsessed with measuring every millilitre you drink.

Instead, learn to recognise the signs that your intake may be falling behind.

A dry mouth, increasing thirst, headache, dizziness, unusual tiredness and urine that becomes noticeably darker or less frequent can all be signs that you need to pay attention to your hydration.

Vomiting or diarrhoea can increase the risk because you are losing fluids while potentially struggling to replace them.

I would much rather a patient contact us early because they are unsure than wait until they are feeling significantly unwell.

When is it more than just a difficult day?

There is an important difference between having a day when you have not quite reached your fluid goal and being unable to drink adequately.

Repeated vomiting, difficulty keeping fluids down, increasing difficulty swallowing or a significant drop in fluid intake should be discussed with your bariatric team.

Some days will require a little more attention

Warm weather, exercise, fever, vomiting and diarrhoea can all increase fluid losses.

Travel can catch people out too.

You may be walking more, sitting on a flight, spending time somewhere warmer or simply be out of your usual routine. If you are travelling after bariatric surgery, think about hydration before you leave rather than waiting until you become thirsty.

Eventually, you stop having to think about it so much

This is perhaps the reassurance I would most like to give someone preparing for bariatric surgery.

The early weeks involve a lot of learning.

Eating feels different. Drinking feels different. Your body is changing and you are trying to remember advice about nutrition, medication, activity and recovery.

It can feel like a lot at first.

But the patients I care for generally find that these new habits become increasingly normal. You learn what works for you. You recognise when you have not had enough to drink. And taking regular fluids becomes something you do without having to plan every sip.

Mounjaro has changed obesity treatment for many people.

Some people who have struggled with their weight for years are experiencing meaningful weight loss with tirzepatide. Their appetite may be easier to manage. Blood sugar control can improve. Everyday activities may become easier.

Then comes a question we hear increasingly often:

Will I have to take Mounjaro for the rest of my life to keep the weight off?

There is no single answer that applies to everyone. What we do know is that obesity is a chronic disease and, for many people, stopping an effective treatment can allow the biological drivers of weight gain to return.

That does not necessarily mean everybody who starts Mounjaro will remain on exactly the same medication or dose for life. It does mean that stopping treatment deserves more thought than simply reaching a particular number on the scales.

What happens when Mounjaro is stopped?

Mounjaro contains tirzepatide. It works on hormonal pathways involved in appetite, food intake and blood glucose regulation.

These effects are present while the medication is being taken. Stopping the medication does not permanently reset the body’s systems for regulating appetite and weight.

We have good clinical evidence showing what this can mean.

In the SURMOUNT-4 trial, people with obesity or overweight received tirzepatide for 36 weeks and achieved an average weight reduction of 20.9%. Participants were then assigned either to continue tirzepatide or switch to placebo for another year.

Those who continued treatment lost further weight. Those who stopped tirzepatide and switched to placebo experienced substantial weight regain.

This is important because it tells us that Mounjaro should not necessarily be viewed in the same way as a short course of antibiotics that is finished once the immediate problem improves.

Obesity usually requires a longer-term approach.

Weight regain does not mean the treatment failed

This point deserves particular attention.

Someone may take Mounjaro, lose a significant amount of weight and then regain some of that weight after stopping.

It can be very easy to interpret this as having gone backwards.

Clinically, that is not necessarily what has happened.

The medication was treating a chronic condition. Once treatment was withdrawn, some of the biological processes involved in that condition became more active again.

We see similar evidence with semaglutide. In an extension of the STEP 1 trial, participants regained around two-thirds of their previous weight loss during the year after semaglutide was stopped.

The lesson from these studies is not that weight-loss medication does not work.

In many patients, it works very well.

The more important lesson is that maintaining the benefit may require ongoing treatment.

Does that mean Mounjaro is a lifelong medication?

Potentially, for some people.

But “lifelong” can be an unhelpful way to think about the decision when somebody is only beginning treatment.

Obesity care can change over time.

A person may respond very well to medication and remain on it long term under medical supervision. Another may need a change in dose or medication. Someone else may stop because of side effects, cost, pregnancy planning or a change in their health.

There are also people living with more severe obesity for whom medication is helpful but does not provide enough improvement in obesity-related health conditions.

The more useful question is therefore:

What is my long-term treatment plan if Mounjaro is working for me?

That conversation should ideally happen before stopping rather than after weight regain has already occurred.

What if you have reached the weight you wanted?

Reaching a healthier weight does not necessarily mean that the underlying tendency towards obesity has disappeared.

This can feel counterintuitive.

We are accustomed to thinking that treatment should finish when the desired result has been achieved. Chronic diseases often work differently.

Blood pressure medication is not usually stopped simply because a person’s blood pressure has improved while taking it. The improvement may be evidence that the treatment is doing its job.

Obesity treatment can require similar long-term thinking.

Current European guidance describes obesity as a chronic, relapsing disease requiring sustained and personalised care. Long-term management should consider not only weight, but also conditions such as type 2 diabetes, cardiovascular disease, sleep apnoea, joint problems and quality of life.

What if you want to stop because of the cost?

This is particularly relevant in Ireland.

Many people are paying privately for weight-loss medication. A monthly expense that feels manageable for six months can look quite different when considered over several years.

Cost is therefore a legitimate part of the treatment conversation.

Stopping solely because the medication has “done its job”, without discussing what happens next, can leave somebody without a plan for maintaining the health improvements they have achieved.

Speak to the clinician managing your treatment before stopping. Depending on your circumstances, the discussion may include continued medical treatment, an alternative medication, nutritional and lifestyle support, or assessment for other obesity treatments.

When is it worth having a conversation about bariatric surgery?

Mounjaro can be a very effective treatment for obesity, but continuing medication indefinitely may not be the right approach for everyone. Some people experience weight regain when treatment stops, while others may still be living with severe obesity or conditions such as type 2 diabetes, sleep apnoea or high blood pressure despite losing weight with medication.

At this point, it may be worth looking beyond the next prescription and considering what treatment could offer a more durable approach to obesity.

Bariatric surgery is one such option. Procedures including gastric sleeve, gastric bypass and SADI-S can achieve substantial, sustained weight loss and improvements in obesity-related health conditions. Surgery also requires long-term commitment, including changes to eating, nutritional supplementation where required and ongoing follow-up, so it is a decision that deserves proper consideration.

Taking Mounjaro does not prevent you from exploring bariatric surgery, and considering surgery does not mean that your previous treatment has failed. It may simply be the next conversation to have about managing obesity over the longer term.

At Blackrock WeightCare, Mr William Robb and the multidisciplinary bariatric team can assess your health, weight history and previous treatment and discuss whether bariatric surgery may be appropriate for you.

Weight loss injections such as Mounjaro, Wegovy and Ozempic have changed the conversation around obesity treatment.

More people than ever are seeking help for their weight, and that’s a positive step. These medications have helped many people lose a meaningful amount of weight, improve their health and take control of conditions such as Type 2 diabetes.

As a result, thousands of people across Ireland are now paying privately for these medications every month.

Before starting treatment, however, there’s one important question that’s often overlooked.

What’s the long-term plan?

It’s easy to focus on the first prescription or the first few months of weight loss. What’s equally important is understanding what happens a year from now, two years from now or even five years from now.

At Blackrock WeightCare, we believe every obesity treatment plan should look beyond the first few kilograms of weight loss. Whether that plan involves medication, bariatric surgery or both, the goal should always be long-term health.

Weight Loss Medications Are Changing Lives

There’s no doubt that GLP-1 medications have transformed obesity care.

For many people, they reduce appetite, improve feelings of fullness and make it easier to adopt healthier eating habits. They can also improve blood sugar control and reduce the risk of obesity-related health complications.

This is why they have become an important treatment option for many patients living with overweight and obesity.

The growing interest in these medications also reflects something positive – people are increasingly recognising that obesity is a medical condition deserving of treatment rather than something that should simply be managed through willpower alone.

The Conversation Shouldn’t Stop at the Prescription

Starting a weight loss medication is one decision.

Planning what happens afterwards is another.

One of the questions we often encourage people to ask is:

“How long am I expected to stay on this medication?”

For some people, long-term treatment may be entirely appropriate.

Others may stop because of side effects, cost, supply issues or personal preference.

Some achieve good weight loss initially but later find their progress slows or their weight begins to increase again after stopping treatment.

These aren’t reasons to avoid medication.

They’re reasons to make sure there’s a longer-term strategy in place from the beginning.

Paying Privately Can Become a Significant Long-Term Commitment

Many people in Ireland currently pay privately for weight loss injections because access through public services or health insurance may be limited.

While the monthly cost may feel manageable at first, obesity is a chronic condition.

Treatment often needs to be considered over years rather than months.

Before committing to ongoing private prescriptions, it’s worth asking yourself:

What is my long-term treatment plan?

If I stop the medication, how will my weight be managed?

What support will I have to maintain my progress?

Are there other treatment options I should understand?

These conversations are just as important as deciding whether to start medication in the first place.

Medications and Bariatric Surgery Are Not Competing Treatments

One of the biggest misconceptions in obesity care is that patients have to choose between weight loss injections and bariatric surgery.

In reality, they are different treatments with different roles.

For some people, medications provide exactly the support they need.

For others, particularly those living with severe obesity or obesity-related health conditions, bariatric surgery may offer the most effective long-term treatment.

There are also patients who benefit from both.

Weight loss medications may be used before surgery to optimise health, after surgery to help manage weight regain in selected cases, or as part of an individual treatment plan where clinically appropriate.

The important point is that treatment should be personalised rather than viewed as an either-or decision.

If Your BMI Is 35 or Above, It’s Worth Understanding All Your Options

Weight loss medications have expanded the treatment choices available for obesity, but they haven’t replaced bariatric surgery.

If your BMI is 35 or above, particularly if you’re living with conditions such as Type 2 diabetes, sleep apnoea or high blood pressure, it’s worth understanding the full range of treatment options before making a long-term decision.

Bariatric surgery remains one of the most effective treatments for severe obesity and has decades of evidence supporting its role in achieving significant, sustained weight loss and improving obesity-related health conditions.

That doesn’t mean surgery is the right choice for everyone.

It does mean it deserves to be part of the conversation.

The Best Treatment Is the One That Fits Your Life

No two people living with obesity have the same journey.

Some have been trying to lose weight for five years.

Others have been living with obesity since childhood.

Some are looking to improve diabetes control, while others simply want to be able to move more comfortably and enjoy everyday life again.

The best treatment plan takes all of these factors into account.

It considers your health, your medical history, your previous weight loss attempts, your lifestyle and your long-term goals.

Most importantly, it looks beyond what happens over the next few months and focuses on what is likely to help you maintain better health for years to come.

Looking Beyond the Next Prescription

Whether you’re already taking Mounjaro, Wegovy or Ozempic, or you’re considering starting one of these medications, it’s worth taking a step back and thinking about the bigger picture.

Obesity is a long-term medical condition, and successful treatment is rarely about a single prescription or a single procedure. It’s about finding an approach that is sustainable, evidence-based and tailored to your individual health needs.

At Blackrock WeightCare, we help patients understand all of the treatment options available for obesity, including gastric sleeve surgery, gastric bypass and SADI-S. Every patient is assessed individually, and treatment recommendations are based on what is most appropriate for their health, circumstances and long-term wellbeing – not a one-size-fits-all approach.

Choosing treatment for obesity is about more than deciding whether to start a medication. It’s about finding an approach that is right for you in the long term. If you’re paying privately for weight loss injections, or thinking about doing so, and your BMI is 35 or above, it’s worth exploring all of the treatment options available before making that commitment. Understanding your options today can help you make a more informed decision about your health for years to come.

It’s one of the first questions many people ask when they begin considering bariatric surgery.

“Is the surgery safe?”

It’s a completely understandable concern. Deciding to have any operation is a significant step, and it’s natural to think about the risks involved.

However, there is another question that deserves equal attention.

What are the risks of continuing to live with severe obesity?

This is an important conversation because obesity is not simply about carrying extra weight. It is a chronic, progressive disease that can affect almost every organ in the body. For many people, the health risks associated with untreated obesity become far greater over time than the risks associated with modern bariatric surgery.

Understanding both sides of the equation is the best way to make an informed decision.

Bariatric Surgery is Very Safe but Every Operation Has Some Risks

No responsible bariatric surgeon will ever tell you that surgery is completely risk-free.

Like any operation, bariatric procedures such as gastric sleeve surgery, gastric bypass and SADI-S carry potential risks but thankfully all bariatric procedures are now relatively low risk operations. Risks can include bleeding, infection, blood clots, leaks, nutritional deficiencies and complications related to anaesthesia.

Before recommending surgery, your bariatric team will discuss these risks with you in detail, explain how they are minimised and answer any questions you may have.

The aim is never to encourage someone towards surgery.

The aim is to help every patient make an informed decision based on their individual health and circumstances.

The Risks of Severe Obesity Continue Every Day

While surgery carries a short-term surgical risk, severe obesity carries a long-term health risk that doesn’t pause while you’re deciding what to do.

Obesity is associated with an increased risk of:

Type 2 diabetes

High blood pressure

Heart disease

Stroke

Obstructive sleep apnoea

Fatty liver disease

Osteoarthritis

Reduced mobility

Certain types of cancer

Many of these conditions develop gradually over several years. Others continue to progress despite medications and lifestyle changes.

This is why obesity is recognised as a chronic medical condition rather than simply a lifestyle issue.

Why Timing Matters

Many people spend years wondering whether they should have bariatric surgery.

During that time, they often continue trying different diets, exercise programmes and, more recently, weight loss medications such as Mounjaro, Wegovy or Ozempic.

These treatments can be very effective for many people and have an important role in obesity management. However, for patients living with severe obesity, particularly those with a BMI of 35 or above and obesity-related health conditions, they may not always provide the long-term results needed to prevent disease progression.

Every year spent living with uncontrolled obesity is another year in which conditions such as diabetes, sleep apnoea or high blood pressure may continue to affect your health.

This doesn’t mean surgery is the right option for everyone.

It does mean that delaying treatment simply because of fear may also carry consequences.

Bariatric Surgery Today Is Very Different to 20 Years Ago

Bariatric surgery has changed significantly over the past two decades.

Advances in minimally invasive surgery, robotic-assisted techniques, and enhanced recovery programmes have all contributed to improved patient outcomes.

Equally important is the way patients are prepared for surgery.

Modern bariatric care doesn’t begin in the operating theatre.

It begins weeks – and often months – before surgery with careful assessment, nutritional optimisation and planning.

This comprehensive approach plays an important role in reducing risk and supporting recovery.

Safety Starts Long Before the Operation

One of the reasons specialist bariatric centres achieve good outcomes is because surgery is only one part of the patient’s journey.

Before an operation is recommended, patients undergo a thorough assessment to understand their overall health, obesity-related conditions, previous treatments and individual needs.

Depending on your circumstances, this may include:

A detailed surgical consultation

Nutritional assessment

Dietetic support

Psychological assessment

A liver reduction diet before surgery

Every stage is designed to ensure that surgery is carried out at the right time and under the safest possible circumstances.

Why Experience Matters

Bariatric surgery is a highly specialised area of medicine, and experience matters.

Patients with severe obesity often have complex medical histories, multiple obesity-related conditions and different surgical considerations compared with other types of abdominal surgery.

At Blackrock WeightCare, care is led by Mr William Robb, Consultant Robotic Bariatric and Upper Gastrointestinal Surgeon, who has helped more than 1,750 patients through their bariatric surgery journey. Working alongside an experienced multidisciplinary team that includes bariatric physicians, specialist nurses, dietitians, psychologists, and allied healthcare professionals, every patient receives an individual treatment plan based on their health, medical history and long-term goals.

This collaborative approach means that every decision – from selecting the most appropriate bariatric procedure to planning recovery and long-term follow-up, is made with patient safety at its centre.

Surgery Is Only One Part of Long-Term Success

One of the biggest misconceptions about bariatric surgery is that the operation itself is the treatment.

In reality, surgery is one part of a much longer journey.

Long-term success depends on regular follow-up, nutritional monitoring, healthy lifestyle habits and ongoing support from an experienced multidisciplinary team.

Whether you’ve had a gastric sleeve, gastric bypass or SADI-S, lifelong follow-up plays an important role in helping you maintain your health and achieve the best possible outcome.

Looking at the Bigger Picture

When people ask whether bariatric surgery is safe, they are usually thinking about the operation itself.

Doctors look at the question slightly differently.

They also consider what may happen if severe obesity continues untreated.

For many patients, bariatric surgery isn’t simply about losing weight.

It’s about reducing the long-term risks associated with obesity, improving existing health conditions and creating the opportunity for a healthier future.

The decision to proceed with surgery is always personal, but it should be made with a full understanding of both the risks of surgery and the risks of ongoing severe obesity.

Making an Informed Decision

If you’re living with severe obesity and wondering whether bariatric surgery is the right option for you, the most important step isn’t deciding on a procedure – it’s having a conversation with an experienced bariatric team.

At Blackrock WeightCare, we’ll take the time to understand your health, discuss your concerns and explain all of the treatment options available to you. Together, we’ll help you make an informed decision based on your individual circumstances and long-term health goals.

Obesity doesn’t affect everyone in the same way.

Two people may have a similar Body Mass Index (BMI), yet their day-to-day lives can look completely different. One person may be managing Type 2 diabetes, sleep apnoea and high blood pressure, while another struggles most with mobility, joint pain or chronic fatigue. Some people have lived with obesity since childhood, while others have seen their weight increase steadily over the years.

This is why treating severe obesity is never as simple as choosing a bariatric procedure.

When your BMI is higher, your treatment needs to be planned around your health, your medical history and the challenges you’re facing—not just the number on the scales.

At Blackrock WeightCare, every patient receives an individual assessment because the right treatment is different for every person.

A Higher BMI Brings Different Challenges

Living with severe obesity can affect almost every aspect of daily life.

Simple activities that many people take for granted can become increasingly difficult. Walking around the supermarket, climbing stairs, getting in and out of a car or enjoying a family holiday may require far more effort than they once did.

As BMI increases, the likelihood of obesity-related conditions also rises. These may include:

Type 2 diabetes

Obstructive sleep apnoea

High blood pressure

Fatty liver disease

Joint pain and reduced mobility

Heart disease

These health conditions often become more difficult to manage over time, making early specialist assessment increasingly important.

Bariatric Surgery Is Never About Choosing the “Best” Procedure

One of the biggest misconceptions about bariatric surgery is that there is one procedure that suits everyone.

There isn’t.

A gastric sleeve may be an excellent option for one patient, while another may benefit more from a gastric bypass or SADI-S. The decision depends on several clinical factors rather than BMI alone.

During your consultation, your surgeon considers much more than your weight.

Your assessment includes:

Your overall health

Existing obesity-related conditions

Previous abdominal or bariatric surgery

Eating habits

Nutritional health

Current medications

Every recommendation is made with one goal in mind – to identify the treatment that offers the safest and most effective long-term outcome for you.

Why Patients With a Higher BMI Often Need a Different Approach

Severe obesity presents unique challenges before, during and after surgery.

Patients with a higher BMI often require more detailed preparation before an operation. This may involve improving nutritional health, managing existing medical conditions, following a liver reduction diet and ensuring every aspect of treatment has been carefully planned.

The operation itself also requires experience.

From anaesthetic planning to the surgical technique and post-operative recovery, every stage of care should be designed around the individual rather than following a standard pathway.

This level of planning is one of the reasons specialist bariatric centres play such an important role in treating complex obesity.

Why SADI-S May Be Recommended for Some Patients

Not every patient with a high BMI requires the same bariatric procedure.

For some people, a gastric sleeve provides an excellent outcome. Others may achieve better long-term results with a gastric bypass.

There are also patients for whom Single Anastomosis Duodeno-Ileal Bypass with Sleeve Gastrectomy (SADI-S) may be the most appropriate option.

SADI-S combines a sleeve gastrectomy with a bypass of part of the small intestine. The procedure is designed to reduce food intake while also producing powerful metabolic changes that can support significant and sustained weight loss and improvements in obesity-related conditions such as Type 2 diabetes.

Like every bariatric procedure, SADI-S is not suitable for everyone. It requires lifelong vitamin supplementation, nutritional monitoring and regular follow-up.

This is why it should only be recommended after a comprehensive assessment by an experienced multidisciplinary bariatric team.

Experience Makes a Difference

High BMI patients often require a different level of assessment and surgical planning. Decisions about the most appropriate procedure are influenced by much more than BMI alone and should take into account each patient’s overall health and obesity-related conditions.

Having dedicated his practice to bariatric surgery for many years, Mr William Robb has extensive experience in treating patients with severe and complex obesity. That experience, together with the support of Blackrock WeightCare’s multidisciplinary team, helps ensure every patient receives a personalised treatment plan based on their individual needs.

Success Isn’t Defined by a Number

People often begin their bariatric journey thinking only about weight loss.

Over time, their priorities usually change.

The conversations during follow-up appointments are rarely about reaching a particular number on the scales.

Instead, patients talk about coming off CPAP treatment, reducing diabetes medication, walking further without stopping, travelling comfortably, fitting into an aeroplane seat, returning to work or simply having the energy to enjoy time with family.

These are the milestones that matter.

They reflect improvements in health, independence and quality of life rather than a specific weight.

The Right Plan Starts With the Right Assessment

Choosing a bariatric procedure should never be based on BMI alone.

Every patient deserves a thorough assessment that looks beyond the scales and considers the complete picture.

At Blackrock WeightCare, your treatment plan is tailored to your individual needs. Our multidisciplinary team carefully evaluates your health, obesity-related conditions, previous treatments and long-term goals before recommending the approach that is most appropriate for you.

For some patients, that may be a gastric sleeve.

For others, it may be a gastric bypass or SADI-S.

The important thing is that the decision is based on clinical expertise and what will provide the safest and most effective outcome for your individual circumstances.

Looking Ahead

Living with severe obesity can affect many aspects of your health and everyday life, but it doesn’t have to define your future. With the right support and a personalised treatment plan, many people experience meaningful improvements in their health, mobility and quality of life.

If you’d like to learn more about the treatment options available for patients with a high BMI, complete our enquiry form or call the team at Blackrock WeightCare on 01 255 2479.

Many people spend months researching bariatric surgery, comparing procedures and deciding whether it’s the right step for them. Only then do they begin to ask an equally important question – what does my health insurance actually cover?

For many, the answer comes as a surprise.

There is a common misconception that bariatric surgery is not covered by private health insurance in Ireland. In reality, many policies from providers such as Vhi, Laya Healthcare, Irish Life Health, Level Health and Garda Medical Aid may provide cover for procedures including gastric sleeve surgery, gastric bypass and SADI-S, provided you meet the medical criteria, your policy includes the appropriate level of cover and the necessary pre-authorisation is obtained.

However, understanding your health insurance is not always straightforward. Waiting periods, policy upgrades, continuity of cover, corporate schemes and insurer-specific approval requirements can all influence when you can have surgery and the level of cover available to you.

At Blackrock WeightCare, we regularly speak to patients who could have avoided unnecessary delays simply by reviewing their policy sooner or understanding how their insurance worked before they were ready for surgery.

The good news is that most of these mistakes are entirely avoidable.

Here are some of the most common ones we see.

Mistake 1 – Assuming Your Health Insurance Doesn’t Cover Bariatric Surgery

One of the biggest misconceptions is that weight loss surgery is never covered by private health insurance.

Many patients dismiss the idea before they have even checked their policy.

In reality, many plans from Ireland’s major health insurers include cover for bariatric surgery when specific medical criteria are met. The exact level of cover varies depending on the policy, hospital access and insurer requirements, which is why it is always worth checking before assuming surgery will need to be self-funded.

A five-minute conversation could completely change your options. Or just fill the form here to know your cover.

Mistake 2 – Waiting Until You’re Ready for Surgery Before Looking at Your Insurance

Many people spend months deciding whether bariatric surgery is right for them.

Only when they decide to proceed do they check their health insurance.

Sometimes they discover that their current policy doesn’t include bariatric surgery benefits.

If they had checked earlier, they may have been able to plan ahead by moving to a more appropriate level of cover and serving any required upgrade waiting period before they wanted surgery.

The earlier you understand your insurance, the more choices you usually have.

Mistake 3 – Not Understanding the Difference Between a New Policy and an Upgrade

This is probably the area that causes the most confusion.

Many people assume that all waiting periods work in exactly the same way.

They don’t.

If you are taking out private health insurance for the first time, or you have allowed your cover to lapse beyond the continuity period, different waiting periods may apply than if you are simply upgrading an existing policy.

Similarly, if you already have health insurance but move to a plan with higher hospital benefits, an upgrade waiting period may apply before you can claim the additional benefits for a pre-existing condition such as obesity. The Health Insurance Authority and the major insurers explain these waiting periods separately because they are not the same thing.

This is why it is important to review your options well in advance rather than waiting until you want surgery immediately.

Mistake 4 – Letting Your Policy Lapse

Life gets busy.

Renewal notices are missed.

Some people cancel their health insurance to save money, planning to restart it later.

Unfortunately, allowing your policy to lapse can have consequences.

If there is a significant break in cover, you may lose continuity of membership and new waiting periods may apply when you take out another policy. In Ireland, insurers generally recognise previous membership if there has not been a break in cover beyond the continuity rules, so maintaining your policy can be very important.

If bariatric surgery is something you may consider in the future, protecting your continuity of cover is often a wise decision.

Mistake 5 – Forgetting That Corporate Health Insurance Can Be Different

Many people receive health insurance through their employer but have never looked closely at what it includes.

Corporate health insurance schemes can differ significantly from individual policies. Depending on the agreement between your employer and the insurer, some schemes may offer different waiting period arrangements, recognise previous cover or provide immediate access to certain benefits that may not be available on a standard individual plan.

In some cases, employees joining a corporate scheme may not have to serve the same waiting periods that would normally apply when taking out a new individual policy. This can mean that, if you meet the medical criteria and your policy includes bariatric surgery, you may be able to access treatment much sooner than you expected. However, these arrangements vary between employers, insurers and individual policy terms, so it is important to check the details of your own cover.

If your health insurance is provided through work, don’t assume it follows the same rules as a standard individual policy. A quick conversation with your insurer or HR department could reveal benefits that you may not have realised were available.

Mistake 6 – Choosing the Cheapest Plan Without Looking at Hospital Cover

When comparing policies, many people naturally focus on the monthly premium.

But the cheapest plan is not always the most suitable.

Hospital access, surgical benefits and procedure cover can vary considerably between plans.

If bariatric surgery may be something you consider over the next few years, understanding exactly what your policy covers is often more valuable than saving a small amount each month.

Mistake 7 – Assuming Switching Insurers Means Starting Again

Many people believe they are “locked in” with one insurer.

That isn’t necessarily true.

If you switch from one insurer to another and maintain continuous cover, the new insurer will generally recognise the time you have already served, provided you meet the continuity rules. If you move to a higher level of cover, however, upgrade waiting periods may still apply to the additional benefits.

Understanding this distinction can save a great deal of confusion.

Mistake 8 – Waiting Too Long to Ask for Help

Perhaps the biggest mistake of all is assuming you need to work everything out yourself.

Health insurance policies can be complex. Hospital lists vary, approval requirements differ between insurers, and understanding waiting periods or the level of cover available under your policy isn’t always straightforward.

The good news is that you don’t have to navigate the process on your own.

At Blackrock WeightCare, our team is happy to help you understand how the insurance process works, what information your insurer may require and what the next steps could look like based on your individual circumstances. While the final decision always rests with your health insurer, we can help make the journey much clearer.

If you have questions about your health insurance or bariatric surgery cover, we’re just a phone call away – 01 255 2479. You can also complete our enquiry form here, and a member of our team will be in touch to guide you through the process and answer any questions you may have.

Sometimes, one conversation is all it takes to understand your options with confidence.

Planning Ahead Can Make the Journey Much Easier

If your BMI is 35 or above, particularly if you are living with type 2 diabetes, sleep apnoea, high blood pressure or other obesity-related conditions, it is worth understanding your health insurance benefits before you decide whether bariatric surgery is right for you.

At Blackrock WeightCare, we regularly help patients navigate the insurance approval process for gastric sleeve surgery, gastric bypass and SADI-S. We can check your level of cover, explain the documentation your insurer may require and manage the pre-authorisation process on your behalf, making the journey as straightforward as possible.

Even if you are not ready to proceed with surgery today, understanding your insurance options now could save valuable time in the future.

Very few people wake up one morning and decide they want weight loss surgery.

For most patients, the decision comes after years of trying to lose weight in different ways. There have been diets, gym memberships, calorie-counting apps, meal replacement programmes and, more recently, weight loss medications such as Mounjaro, Wegovy or Ozempic.

Some approaches may have worked for a while. Others may not have made much difference at all. But for many people living with obesity, one thing remains the same – the weight keeps coming back, and with it comes the frustration of feeling like they’re starting over again.

At Blackrock WeightCare, we often meet patients who tell us, “I wish I’d come to see someone sooner. I didn’t realise there were so many treatment options available.”

Here are five signs that it may be time to explore whether bariatric surgery could be part of your treatment plan.

1. Your Life Is Starting to Revolve Around Your Weight

Obesity is rarely just about the number on the scales.

At first, it might be avoiding certain clothes or feeling uncomfortable in photographs. Then you notice you’re choosing restaurants based on seating, worrying about fitting into an aeroplane seat or avoiding activities because you’re concerned about keeping up.

Some people stop walking with friends because their knees hurt. Others avoid holidays because they find travelling exhausting. Playing with children or grandchildren becomes more difficult, and everyday tasks leave them short of breath.

These are often the moments when people realise that obesity is affecting much more than their appearance.

Weight loss surgery isn’t about changing how you look. It’s about helping you get back to living the life you want.

2. You’ve Lost Weight Before… But You Always Gain It Back

Almost every patient who walks through our doors has lost weight before.

Some have lost two or three stone. Others have lost much more.

The problem isn’t losing weight.

The problem is keeping it off.

Obesity changes the way the body regulates hunger and metabolism. After weight loss, appetite hormones increase while the body burns fewer calories, making weight regain much more likely. This is one of the reasons why obesity is now recognised as a chronic disease rather than simply a lifestyle issue.

This is also why repeated dieting can become so discouraging.

Weight loss medications such as Mounjaro, Wegovy and Ozempic have changed obesity treatment by helping to regulate appetite and improve blood sugar control. For many patients, they are an effective part of treatment.

However, for people living with severe obesity, particularly those with a BMI of 35 or above and obesity-related health conditions, medication alone may not provide the degree of weight loss needed to improve long-term health.

That is where bariatric surgery becomes part of the conversation.

3. Your GP Is Treating More Than Just Your Weight

One prescription becomes two.

Blood pressure tablets are added.

Your blood sugar starts creeping up.

You are referred for a sleep study.

Your knees hurt more than they used to.

Perhaps you’ve been told you have fatty liver disease or prediabetes.

Many people don’t realise that these conditions are closely linked to obesity.

By the time obesity has begun affecting your heart, liver, joints or pancreas, treatment is no longer just about losing weight. It is about preventing further damage and, where possible, reversing disease.

This is why current international guidelines recommend considering bariatric surgery for people with a BMI of 35 or above, particularly when obesity-related conditions such as type 2 diabetes, sleep apnoea or high blood pressure are present.

The earlier obesity is treated, the greater the opportunity to improve long-term health.

4. You’re Wondering Whether Mounjaro, Wegovy or Ozempic Will Be Enough

This is one of the most common questions patients ask today.

The answer is different for everyone.

For some people, weight loss medication is exactly the right treatment. It can reduce appetite, support meaningful weight loss and improve metabolic health.

For others, particularly those living with severe obesity, medication may be an important first step but not the complete solution.

Rather than asking whether medication is “better” than surgery, a more helpful question is:

What treatment is most likely to improve my health in the long term?

Sometimes the answer is medical weight management.

Sometimes it is bariatric surgery.

Sometimes the best outcomes come from combining both approaches as part of a structured treatment plan.

At Blackrock WeightCare, we believe patients deserve to understand every evidence-based option before making that decision.

5. You’ve Started Thinking “Maybe I Should Have Done Something Sooner”

This thought is more common than people realise.

Many patients tell us they spent years putting off a consultation because they felt surgery was a last resort.

They wanted to lose one more stone.

Try one more diet.

Wait for a new medication.

See if next year would be different.

There is nothing wrong with wanting to explore every option available.

But obesity doesn’t wait.

While you are deciding what to do, type 2 diabetes can progress. Blood pressure may become harder to control. Fatty liver disease can worsen, and joint damage may become more difficult to reverse.

Having a consultation does not mean you are committing to surgery.

It simply gives you the opportunity to understand your options before obesity has a greater impact on your health.

It’s Not About Choosing Bariatric Surgery Over Weight Loss Medication

One of the biggest misconceptions about obesity treatment is that patients have to choose between weight loss medication and bariatric surgery.

In reality, modern obesity care is far more personalised.

Some patients achieve excellent results with medical weight management and medications such as Mounjaro, Wegovy or Ozempic.

Others benefit most from bariatric procedures such as gastric sleeve surgery, gastric bypass or SADI-S, particularly when they have severe obesity or obesity-related medical conditions.

The right treatment depends on your BMI, your medical history, your previous weight loss attempts and, most importantly, your long-term health goals.

The Conversation Could Change Your Future

If you recognised yourself in one or more of these signs, it does not necessarily mean that weight loss surgery is the right option for you.

What it does mean is that it may be time to have a conversation with a specialist bariatric team. Understanding your options early can help you make an informed decision before obesity has a greater impact on your health.

At Blackrock WeightCare, we specialise in bariatric surgery for patients living with obesity. Every patient is assessed individually, taking into account their BMI, medical history, obesity-related health conditions and previous weight loss journey. Our multidisciplinary team will guide you through the available surgical options, including gastric sleeve, gastric bypass and SADI-S, and help determine which procedure is most appropriate for your individual needs.

If your BMI is 35 or above and obesity is affecting your health or quality of life, now may be the right time to explore whether bariatric surgery could help you achieve lasting improvements in your health.
