For many people living with obesity, the decision to have weight loss surgery comes after years of struggle. But it can be incredibly disheartening to finally take that step — only to be told “your BMI is too high for surgery.”

At Blackrock WeightCare, we understand how discouraging that can feel. But with us, the story doesn’t end with no.
We believe that everyone — no matter their starting point — deserves safe, compassionate, and expert care. And for high-BMI patients, that’s exactly what we provide.

Why Some Centres Turn High-BMI Patients Away

Across Europe, many bariatric centres set strict BMI limits for surgery. These limits are often based on resources, equipment, or experience rather than patient potential. Operating on high-BMI patients does require additional expertise, planning, and advanced technology — but it’s absolutely possible when done in the right hands.

Unfortunately, this means many patients are told they can’t have surgery when, in reality, they simply need a team with the right experience to support them safely.

That’s where Blackrock WeightCare comes in.

Europe’s Leading Expertise in Robotic Bariatric Surgery

At the heart of Blackrock WeightCare’s program is Mr William Robb, one of Europe’s highest-volume Robotic Bariatric Surgeon, recognised for his extensive experience in performing complex and high-BMI surgeries — including the SADI-S procedure.

SADI-S (Single Anastomosis Duodenal–Ileal Bypass with Sleeve) is one of the most advanced forms of bariatric surgery available today.
It’s especially effective for patients who have:

A higher BMI – particularly those with a BMI over 50

Weight regain after previous surgery – particularly weight regain after previous sleeve gastrectomy

Metabolic conditions such as diabetes or PCOS

The SADI-S procedure offers two levels of benefit — restriction (by creating a smaller stomach) and malabsorption (by rerouting part of the intestine).
This combination promotes powerful and sustainable weight loss, often leading to significant improvements in diabetes, blood pressure, and hormonal balance.

William Robb has recently built one of the highest volume robotic SADI-S practices in Europe using robotic precision to enhance safety, reduce recovery time, and improve long-term outcomes — even for patients previously considered “too high-risk” elsewhere.

Safety First — Always

Safety is at the centre of everything we do at Blackrock WeightCare.
Our approach includes:

Comprehensive pre-surgical assessments tailored for high-BMI patients

State-of-the-art robotic surgical systems to improve precision and reduce complications

A highly experienced multidisciplinary team including endocrinologists, dietitians, anaesthetists, and specialist nurses

Close post-operative monitoring to ensure a smooth and supported recovery

We don’t just perform surgery — we create a pathway that allows our patients to heal, progress, and thrive.

A Support System That Lasts

Our care doesn’t end after surgery. Every patient is part of our two-year structured Aftercare Program, which includes:

Regular follow-up appointments and medical reviews

Personalised diet and supplement advice from our expert dietitians

24/7 clinical access during recovery

Emotional and lifestyle support

A dedicated SADI-S WhatsApp community where patients connect, share experiences, and motivate one another

This network of professional and peer support helps ensure that every patient — no matter where they start — has the best chance at long-term success.

A Second Chance at Health — Safely in Ireland

If you’ve been told that your BMI is too high for surgery, or that your case is too complex, please know there are still safe and proven options.
At Blackrock WeightCare, under the care of Mr William Robb, patients across Ireland and Europe have achieved transformative results with advanced procedures such as SADI-S.

If you’d like to learn more about how SADI-S surgery can help you achieve lasting results — even if other centres have said no — you can explore the full procedure and patient information here: Robotic SADI-S at Blackrock WeightCare

The day I tried on my communion dress again, I cried. Not because it fit – but because it was loose.

That small white dress, tucked away for almost two decades, became the moment I truly saw how far I’d come. It wasn’t just fabric and thread – it was proof. Proof that the girl who once hid behind layers of clothes and self-doubt had reclaimed her life.

But let me take you back to where it began.

In June 2024, I made a decision that would change everything. I was 26 stone, uncomfortable in my own body, and exhausted by the smallest things – climbing stairs, standing all day to teach, pretending to feel okay when I wasn’t. I had just qualified as a teacher, a job I’d dreamed of, but every evening I came home drained, my energy gone long before the day was over.

Somewhere deep down, I knew I was surviving, not living.

I started reading about bariatric surgery – cautiously, secretly, the way you search for something you’re afraid to believe might actually help. That’s when I found Blackrock WeightCare.

From the first moment I spoke to Mr. William Robb, something shifted. He didn’t speak in numbers or quick fixes. He spoke with understanding – about health, confidence, and the life waiting on the other side of fear. I was only 23 and nervous about surgery, but he made me feel safe. He made me believe that my body wasn’t broken; it just needed help.

Over the next six months, the team guided me through everything — physically, emotionally, and mentally. I learned what to expect, how to prepare, and most importantly, that this journey wasn’t about losing weight; it was about finding myself again.

In the end, we decided that a Mini Gastric Bypass was right for me. And on the day of surgery, I wasn’t scared anymore. I was ready.

Everything went perfectly. No complications, no panic — just calm, steady care. And after that, came the magic of small changes stacking up into something extraordinary.

The aftercare team became my compass — regular check-ins, gentle reminders, and the kind of encouragement that keeps you grounded when the mirror starts to show a stranger you’re still getting to know.

Slowly, the stairs got easier. The clothes I liked started to fit. The reflection in the mirror began to smile back.

Then one afternoon, on a whim, I went to my wardrobe and found the box with my communion dress. I don’t even know why. Maybe I just wanted to see it — a little symbol of the girl I used to be.

I held my breath as I pulled it over my head.
It didn’t just fit. It floated.

I stood there in front of the mirror, and for the first time in years, I saw myself — strong, proud, and free.

From 26 stone to 14 stone 8 lbs, I lost 160 lbs – but what I gained is so much more.
I gained my life.

This journey wasn’t about changing how I look. It was about changing how I live. And thanks to Mr. Robb and the incredible team at Blackrock WeightCare, I finally feel like I’m living the life that was waiting for me all along.

For many people, gastric sleeve surgery marks a life-changing step toward better health. It reduces hunger, limits portion sizes, and helps patients lose a significant amount of weight — often improving conditions like diabetes, sleep apnoea, and high blood pressure.

But for some, results may slow down over time. Weight loss might plateau, or weight might increase even with continued effort. If this sounds familiar, you’re not alone — and it doesn’t mean your sleeve has failed. It may simply mean that your body needs a different approach to continue progressing. That’s where SADI-S revision surgery comes in.

Understanding the Gastric Sleeve

During a gastric sleeve (or sleeve gastrectomy), around 75–80% of the stomach is removed, leaving a smaller, sleeve-shaped pouch. This limits how much you can eat and reduces hunger hormones, helping with early and effective weight loss.

However, the procedure doesn’t significantly alter the way your body absorbs nutrients and calories. Over time, metabolism can adapt, and the same reduced portions may no longer result in further weight loss.

Why Some Patients Struggle After Gastric Sleeve Surgery

There are several reasons some patients find their results taper off or weight regain becomes an issue:

Metabolic adaptation: The body becomes more efficient at using energy, meaning you burn fewer calories.

Stretching of the sleeve: Over time, the stomach may expand slightly, allowing larger portions.

Underlying hormonal factors: Conditions like PCOS, insulin resistance, or thyroid imbalance can affect long-term results.

Weight regain or plateau: Even with healthy eating, the body’s energy balance can shift, leading to stalled progress.

If any of these apply, SADI-S (Single Anastomosis Duodenal-Ileal Bypass with Sleeve) may be the next step to help achieve sustainable results.

What is SADI-S Revision Surgery?

SADI-S is a surgical advancement that builds on the original sleeve gastrectomy.
It combines the restrictive benefits of the sleeve with an intestinal bypass, meaning the body absorbs fewer calories and nutrients.

In simple terms, it helps reduce both how much you eat and how much you absorb — offering dual benefits for long-term weight loss and metabolic health.

Benefits of Converting from Gastric Sleeve to SADI-S

Enhanced weight loss: Many patients who had stopped losing weight after sleeve surgery experience significant additional loss after SADI-S.

Improved diabetes control: SADI-S has shown strong results in improving or even resolving type 2 diabetes.

Better long-term results: Combining restriction and malabsorption reduces the risk of weight regain.

Hormonal balance: Patients often notice improved energy, reduced hunger, and better management of hormonal conditions such as PCOS.

Improved cholesterol and blood pressure: Metabolic improvements are common within months.

Why Choose Blackrock WeightCare for SADI-S Revision

At Blackrock WeightCare, revision surgery is approached with the same precision, compassion, and care as your first procedure.
He has extensive experience in SADI-S and complex revision procedures, ensuring you are in the safest hands possible.

The SADI-S procedure (Single Anastomosis Duodeno-Ileal Bypass with Sleeve Gastrectomy) is a powerful bariatric surgery option that combines restriction of food intake with a degree of malabsorption. While it can deliver excellent weight-loss and metabolic results, the changes it creates in your digestive system mean that nutrition must be carefully managed for the rest of your life.

Getting your diet right after SADI-S is not just about avoiding deficiencies—it’s about giving your body the right fuel to heal, function well, and sustain your long-term health.

Why Nutrition is Different After SADI-S

The SADI-S procedure involves two key changes:

A smaller stomach (sleeve gastrectomy) – This limits the amount of food you can eat at one time.

A bypass of part of the small intestine – This reduces calorie and nutrient absorption.

Because of these changes, patients are at greater risk of nutrient deficiencies than with restrictive procedures alone. Lifelong attention to diet, hydration, and supplementation is essential.

Core Nutritional Priorities After SADI-S

1. Protein Intake

Protein supports healing, muscle preservation, and energy. After SADI-S, your smaller stomach means you need to prioritise protein at every meal.

Target: 80–100g per day (as recommended by your dietitian)

Sources: Lean meats, poultry, fish, eggs, low-fat dairy, tofu, legumes

2. Vitamin and Mineral Supplementation

The bypassed section of intestine reduces absorption of many nutrients. Without supplementation, deficiencies can develop quickly.
Commonly required supplements include:

Multivitamin and mineral complex (high potency)

Vitamin B12 – often via injections or high-dose oral/sublingual forms

Iron – especially important for menstruating women

Calcium citrate with Vitamin D

Fat-soluble vitamins (A, D, E, K) – due to reduced fat absorption

3. Hydration

With reduced stomach capacity, dehydration can happen easily.

Sip water regularly throughout the day

Avoid drinking with meals to prevent discomfort

Aim for at least 1.5–2 litres daily

4. Balanced Micronutrients

Even with supplements, it’s important to eat a variety of foods:

Fresh vegetables and fruits (for fibre, antioxidants, and phytonutrients)

Whole grains in small portions for slow-release energy

Healthy fats from sources like olive oil, avocado, and oily fish

5. Meal Structure

Because of the smaller stomach and altered absorption:

Eat 4–6 small meals a day

Chew food thoroughly and eat slowly

Focus on nutrient-dense foods—avoid “empty calories” from sugary drinks, sweets, or fried foods

6. Regular Blood Tests

Nutrient levels can change over time, even if you feel well. Scheduled blood tests help identify and correct deficiencies early.

Long-Term Lifestyle Habits

Nutrition after SADI-S isn’t just about the first year—it’s a lifelong commitment. Patients who succeed long-term tend to:

Follow their supplement plan daily

Keep protein as the main focus of meals

Limit alcohol and ultra-processed foods

Attend regular follow-up appointments

Personalised Advice Matters

While these are general nutritional considerations after SADI-S surgery, every patient’s needs are unique. Factors such as age, gender, existing health conditions, activity level, and personal goals will affect what’s right for you.

That’s why at Blackrock WeightCare, nutritional guidance is a core part of our structured aftercare programme.
Asthma can be particularly challenging to manage in individuals struggling with obesity. The extra weight not only intensifies asthma symptoms but complicates their overall management. Here we will explore the unique challenges faced by obese patients with asthma, delve into the triggers exacerbated by excess weight, and discuss how bariatric surgery can serve as a potent solution for these intertwined health issues.

Understanding the Asthma-Obesity Connection

The link between obesity and asthma is profound and complex. Excess body weight exacerbates asthma symptoms through increased inflammation and mechanical strain on the lungs. Obese individuals often suffer from more frequent and severe asthma episodes, complicating the management of their condition. This underscores the urgent need for specialized treatment strategies tailored to address the specifics of asthma in the context of obesity.

Identifying Asthma Triggers Aggravated by Obesity

Obese patients may face specific asthma triggers that are less significant in non-obese individuals:

Physical Inactivity: Reduced mobility can impair lung function and worsen asthma symptoms.

Gastroesophageal Reflux Disease (GERD): Commonly found in obese individuals, GERD can exacerbate asthma by causing acid reflux that irritates the airways.

Inflammation: Increased body fat in obesity amplifies systemic inflammation, potentially intensifying asthma symptoms.

Allergens: Obesity may modify immune responses to allergens, heightening sensitivity to environmental triggers such as pollen, dust, and pet dander.

Challenges of Asthma Management in Obesity

The physiological changes brought about by obesity complicate traditional asthma treatments, such as diminished lung capacity and heightened airway responsiveness. Managing asthma in obese patients typically requires a comprehensive strategy that addresses both the respiratory symptoms and the underlying obesity.

Strategic Management of Asthma in Obese Patients

Effective asthma management in obese individuals is intricately linked to successful weight management:

Improving Respiratory Symptoms: Weight loss has been shown to enhance lung function, reduce inflammation, and decrease the frequency of asthma attacks.

Boosting Medication Efficacy: Weight reduction can improve the body’s response to asthma medications by reducing systemic inflammation and enhancing lung function.

The Role of Bariatric Surgery

Bariatric surgery offers a potent intervention for those grappling with both severe obesity and asthma. By enabling significant weight loss, bariatric surgery not only helps improve asthma control but also alters metabolic outcomes that directly affect respiratory health:

Weight Loss Impact on Asthma: Substantial weight reduction following bariatric surgery is associated with better asthma control and a reduced dependency on medications.

Inflammation Reduction: Bariatric procedures are known to decrease systemic inflammation, a common factor in both obesity and asthma.

Lifestyle Modifications: Post-surgery, many patients become more active, which further assists in managing asthma symptoms.

For obese patients with asthma, it is crucial to understand and effectively manage the triggers that exacerbate their condition. Bariatric surgery is not just a method for weight loss; it represents a comprehensive approach to addressing the combined challenges of obesity and asthma. It is advisable for patients to consult with healthcare providers to determine if bariatric surgery is suitable for their specific health needs.

Individuals struggling with obesity-related asthma are encouraged to explore bariatric surgery as a potential treatment option. Scheduling a consultation with a specialist at Blackrock WeightCare can provide more information about available options and start a journey towards a healthier, more active life, free from the constraints of asthma exacerbated by obesity.

Knee surgery is a crucial intervention for patients suffering from severe knee problems, including arthritis and injury-induced damage. However, for super-obese patients, traditional knee surgery presents unique challenges that can complicate both the procedure and the recovery process.

This blog explores the obstacles associated with performing knee surgery on patients living with obesity and discusses how bariatric surgery can serve as a preliminary step to enhance the effectiveness and safety of orthopaedic operations.

Understanding the Challenges

Super obese patients — patients with a body mass index (BMI) of 40 or higher—face several specific challenges when undergoing knee surgery:

Increased Surgical Risks: The physical size and weight of patients with a BMI of over 40 can make surgery technically more difficult. The excess weight puts additional stress on the knee joint, making surgery more difficult and increasing the risk of complications such as infections, blood clots, and prolonged healing times.

Anaesthetic Complications: Anaesthesia in patients living with severe and complex obesity is more complex due to issues with drug dosing and the potential for respiratory problems. These patients often have a reduced lung capacity and may suffer from obstructive sleep apnoea, which can complicate postoperative recovery.

Postoperative Complications: Super obese patients are at a higher risk of post-surgical complications. Their weight can impede mobility, compliance with physiotherapy and rehabilitation, essential components of successful knee surgery recovery. Reduced mobility increases the risk of deep vein thrombosis (DVT) and pulmonary embolism, which can potentially be life-threatening.

Limited Rehabilitation Success: The effectiveness of postoperative physical therapy is crucial for the success of knee surgery. Obese patients may struggle with the physical therapy regime which can affect the ultimate success of the surgery.

How Bariatric Surgery Can Pave the Way for Successful Knee Surgery

Bariatric surgery offers a viable solution to reduce these risks by facilitating significant weight loss prior to knee surgery. The benefits of undergoing bariatric surgery before knee or other joint replacement orthopaedic procedures include:

Reduced Body Weight: Significant weight loss following bariatric surgery reduces the stress on the knees, thereby lowering the surgical risk and potentially improving the outcomes of knee surgery.

Improved Overall Health:Bariatric surgery can lead to improvements in many obesity-related comorbidities such as type 2 diabetes, hypertension, cardiovascular diseases, obstructive sleep apnoea and well-being which are critical for reducing overall surgical risks.

Enhanced Mobility: Weight loss improves mobility and flexibility, which can make the rehabilitation process more manageable, effective and successful.

Lower Risk of Surgical Complications: The reduction in weight can decrease the chances of complications related to anesthesia, as well as postoperative complications such as wound and joint infections and DVT.

For super-obese patients, the path to successful knee surgery often begins with addressing the underlying issue of obesity. Bariatric surgery not only provides a means to achieve significant weight loss but also enhances the safety and effectiveness of subsequent orthopaedic procedures. By reducing the body weight and improving overall health, bariatric surgery helps pave the way for successful knee surgery.

If you or someone you know is struggling with the obesity and is considering knee surgery, it may be beneficial to explore bariatric surgery as a first step. Consulting with a specialist at Blackrock WeightCare will help you understand weightloss/bariatric surgery. It can provide you with a comprehensive treatment plan tailored to your specific needs.

Blackrock WeightCare, a leader in innovative bariatric surgery solutions, is excited to announce the launch of the SADI-S procedure in Ireland. This significant advancement in bariatric surgery is led by Consultant Upper Gastrointestinal Surgeon, Mr. William Robb M.D., F.R.C.S.I., who brings this cutting-edge technique to Irish patients for the first time.

The SADI-S (Sleeve Gastrectomy with Duodenal Intestinal Bypass Single Anastomosis) procedure combines the benefits of sleeve gastrectomy with a partial intestinal bypass. Designed primarily for patients with a BMI over 45 or those suffering from severe obesity-related health issues, the procedure promises a transformative solution to effectively manage and treat obesity.

Mr. Robb, a pioneer in minimally invasive and robotic stomach and esophageal surgery, utilizes the latest in robotic surgical technology to perform the SADI-S procedure. His expertise ensures that patients receive top-tier surgical care with an emphasis on safety and precision.

“Bringing the SADI-S procedure to Ireland marks a revolutionary advancement in our surgical offerings,” explains Mr. Robb. “This procedure appears to offer stronger weight loss outcomes for patients living with obesity. The SADI-S procedure also seems to give very enduring results with at least as effective outcomes for patients with diabetes and other metabolic complications of their obesity when compared to traditional bariatric surgeries such as sleeve gastrectomy and roux-en-y gastric bypass. It represents a very strong surgical option and hope for those battling severe obesity.”

One of the key benefits of the SADI-S procedure is its ability to maintain the integrity of the stomach’s exit valve (the pylorus), reducing the risk of post-surgical complications such as dumping syndrome and reflux. This factor contributes to a smoother and more comfortable recovery process for patients. The procedure also shows substantial improvements in obesity-related conditions, such as diabetes, thereby enhancing overall patient health and quality of life.

Blackrock WeightCare is dedicated to supporting patients throughout their weight loss journey with comprehensive aftercare programs designed to ensure long-term success and well-being.

The SADI-S procedure is particularly beneficial for patients who have previously undergone sleeve gastrectomy but either had suboptimal weight loss or significant weight regain after this procedure. Mr. Robb and his team are committed to providing these patients with renewed opportunities for substantial weight loss and improved health.

“We are proud to offer the SADI-S procedure as part of our commitment to pioneering significant advancements in the field of bariatric surgery,” says Mr. Robb. “Our goal at Blackrock WeightCare is to empower our patients with innovative treatments that transform lives and foster a healthier future.”

Is the SADI procedure an option for you?

The single-anastomosis duodenal switch (SADI) procedure has emerged as a viable surgical option for patients who have experienced a failed or poor outcome after sleeve gastrectomy.

Sleeve gastrectomy has become the operation of choice for many patients and surgeons but it has some limitations and some patients may experience insufficient weight loss or weight regain after the procedure. In such cases, the SADI procedure can offer a potential solution.

The SADI procedure combines aspects of both restrictive and malabsorptive surgeries, making it a powerful tool in treating morbid obesity. It involves creating a single anastomosis between the duodenum (the first part of the intestine after the stomach) and the ileum. This allows food to bypass a length of the small intestine and results in reduced absorption of calories and nutrients, leading to significant weight loss.

When a patient experiences inadequate weight loss or weight regain after a sleeve gastrectomy, several factors may contribute to these outcomes. These can include the dilation of the sleeve over time, the restoration of the stomach’s capacity, and a decrease in the restrictive effect of the surgery. In such cases, the SADI procedure can address these issues by enhancing the malabsorptive component and providing a more powerful metabolic effect.

The SADI procedure offers several advantages for patients who have experienced a poor outcome after a sleeve gastrectomy. First, it allows for a greater degree of weight loss compared to simply a revisional sleeve gastrectomy which redoes the original operation with the same risk of poor outcome or of weight regain.

By increasing the malabsorptive component of the surgery, a SADI provides a more substantial metabolic effect, resulting in enhanced weight loss and resolution of comorbidities. Moreover, at Blackrock WeightCare the SADI procedure is performed using a robotic surgery, which reduces surgical risks by improving the accuracy of surgery and improves patient recovery. SADI is therefore an appealing option for patients seeking a revisional procedure after a failed sleeve gastrectomy. It is important to note that the decision to undergo a SADI procedure after a failed sleeve gastrectomy should be carefully considered and individualized for each patient. A thorough evaluation of the patient’s medical history, current weight, comorbidities, and overall health should be conducted by a multidisciplinary team of healthcare professionals, including bariatric surgeons,
dieticians, and psychologists.

Additionally, postoperative follow-up and long-term management play a crucial role in the success of the SADI procedure. Patients should adhere to a strict dietary plan, engage in regular physical activity, and attend regular check-ups to monitor their progress and address any potential complications. Ongoing support through Blackrock WeightCare’s Aftercare Program provides valuable guidance and motivation throughout the weight loss journey.

In conclusion, the SADI procedure offers a valuable option for patients who have experienced inadequate weight loss or weight regain after a sleeve gastrectomy. It provides a powerful metabolic effect and enhances weight loss outcomes. However, careful patient selection, thorough evaluation, and long-term follow-up are essential to ensure the success of the procedure. With appropriate patient selection and comprehensive postoperative care, the SADI procedure can be an effective tool in achieving sustainable weight loss and improving the health of patients who have experienced a failed sleeve gastrectomy.
