Very few people wake up one morning and decide they want weight loss surgery.

For most patients, the decision comes after years of trying to lose weight in different ways. There have been diets, gym memberships, calorie-counting apps, meal replacement programmes and, more recently, weight loss medications such as Mounjaro, Wegovy or Ozempic.

Some approaches may have worked for a while. Others may not have made much difference at all. But for many people living with obesity, one thing remains the same – the weight keeps coming back, and with it comes the frustration of feeling like they’re starting over again.

At Blackrock WeightCare, we often meet patients who tell us, “I wish I’d come to see someone sooner. I didn’t realise there were so many treatment options available.”

Here are five signs that it may be time to explore whether bariatric surgery could be part of your treatment plan.

1. Your Life Is Starting to Revolve Around Your Weight

Obesity is rarely just about the number on the scales.

At first, it might be avoiding certain clothes or feeling uncomfortable in photographs. Then you notice you’re choosing restaurants based on seating, worrying about fitting into an aeroplane seat or avoiding activities because you’re concerned about keeping up.

Some people stop walking with friends because their knees hurt. Others avoid holidays because they find travelling exhausting. Playing with children or grandchildren becomes more difficult, and everyday tasks leave them short of breath.

These are often the moments when people realise that obesity is affecting much more than their appearance.

Weight loss surgery isn’t about changing how you look. It’s about helping you get back to living the life you want.

2. You’ve Lost Weight Before… But You Always Gain It Back

Almost every patient who walks through our doors has lost weight before.

Some have lost two or three stone. Others have lost much more.

The problem isn’t losing weight.

The problem is keeping it off.

Obesity changes the way the body regulates hunger and metabolism. After weight loss, appetite hormones increase while the body burns fewer calories, making weight regain much more likely. This is one of the reasons why obesity is now recognised as a chronic disease rather than simply a lifestyle issue.

This is also why repeated dieting can become so discouraging.

Weight loss medications such as Mounjaro, Wegovy and Ozempic have changed obesity treatment by helping to regulate appetite and improve blood sugar control. For many patients, they are an effective part of treatment.

However, for people living with severe obesity, particularly those with a BMI of 35 or above and obesity-related health conditions, medication alone may not provide the degree of weight loss needed to improve long-term health.

That is where bariatric surgery becomes part of the conversation.

3. Your GP Is Treating More Than Just Your Weight

One prescription becomes two.

Blood pressure tablets are added.

Your blood sugar starts creeping up.

You are referred for a sleep study.

Your knees hurt more than they used to.

Perhaps you’ve been told you have fatty liver disease or prediabetes.

Many people don’t realise that these conditions are closely linked to obesity.

By the time obesity has begun affecting your heart, liver, joints or pancreas, treatment is no longer just about losing weight. It is about preventing further damage and, where possible, reversing disease.

This is why current international guidelines recommend considering bariatric surgery for people with a BMI of 35 or above, particularly when obesity-related conditions such as type 2 diabetes, sleep apnoea or high blood pressure are present.

The earlier obesity is treated, the greater the opportunity to improve long-term health.

4. You’re Wondering Whether Mounjaro, Wegovy or Ozempic Will Be Enough

This is one of the most common questions patients ask today.

The answer is different for everyone.

For some people, weight loss medication is exactly the right treatment. It can reduce appetite, support meaningful weight loss and improve metabolic health.

For others, particularly those living with severe obesity, medication may be an important first step but not the complete solution.

Rather than asking whether medication is “better” than surgery, a more helpful question is:

What treatment is most likely to improve my health in the long term?

Sometimes the answer is medical weight management.

Sometimes it is bariatric surgery.

Sometimes the best outcomes come from combining both approaches as part of a structured treatment plan.

At Blackrock WeightCare, we believe patients deserve to understand every evidence-based option before making that decision.

5. You’ve Started Thinking “Maybe I Should Have Done Something Sooner”

This thought is more common than people realise.

Many patients tell us they spent years putting off a consultation because they felt surgery was a last resort.

They wanted to lose one more stone.

Try one more diet.

Wait for a new medication.

See if next year would be different.

There is nothing wrong with wanting to explore every option available.

But obesity doesn’t wait.

While you are deciding what to do, type 2 diabetes can progress. Blood pressure may become harder to control. Fatty liver disease can worsen, and joint damage may become more difficult to reverse.

Having a consultation does not mean you are committing to surgery.

It simply gives you the opportunity to understand your options before obesity has a greater impact on your health.

It’s Not About Choosing Bariatric Surgery Over Weight Loss Medication

One of the biggest misconceptions about obesity treatment is that patients have to choose between weight loss medication and bariatric surgery.

In reality, modern obesity care is far more personalised.

Some patients achieve excellent results with medical weight management and medications such as Mounjaro, Wegovy or Ozempic.

Others benefit most from bariatric procedures such as gastric sleeve surgery, gastric bypass or SADI-S, particularly when they have severe obesity or obesity-related medical conditions.

The right treatment depends on your BMI, your medical history, your previous weight loss attempts and, most importantly, your long-term health goals.

The Conversation Could Change Your Future

If you recognised yourself in one or more of these signs, it does not necessarily mean that weight loss surgery is the right option for you.

What it does mean is that it may be time to have a conversation with a specialist bariatric team. Understanding your options early can help you make an informed decision before obesity has a greater impact on your health.

At Blackrock WeightCare, we specialise in bariatric surgery for patients living with obesity. Every patient is assessed individually, taking into account their BMI, medical history, obesity-related health conditions and previous weight loss journey. Our multidisciplinary team will guide you through the available surgical options, including gastric sleeve, gastric bypass and SADI-S, and help determine which procedure is most appropriate for your individual needs.

If your BMI is 35 or above and obesity is affecting your health or quality of life, now may be the right time to explore whether bariatric surgery could help you achieve lasting improvements in your health.

Bariatric surgery has emerged as a transformative solution for individuals struggling with severe obesity and related health conditions. In Ireland, there are several types of bariatric surgery available, each tailored to meet different needs and medical conditions. Here’s an in-depth look at the various bariatric surgery options offered at Blackrock WeightCare, guided by the expertise of Dr. William Robb.

Gastric Bypass Surgery

What is Gastric Bypass Surgery?

Gastric bypass, also known as Roux-en-Y gastric bypass, is a procedure that involves creating a small pouch at the top of the stomach and connecting it directly to the small intestine. This reduces significantly the capacity of the stomach and bypasses a segment of the small intestine resulting in reduced absorption.

Benefits

Gastric bypass surgery leads to significant weight loss, with patients typically losing 60-80% of their excess weight. This procedure also helps in resolving or improving conditions such as type 2 diabetes, hypertension, and sleep apnea. One of the major advantages of gastric bypass is its long-term success in maintaining weight loss. By altering the digestive tract, this surgery changes how the body absorbs nutrients, leading to sustained weight loss over the years.

Sleeve Gastrectomy

What is Sleeve Gastrectomy?

Sleeve gastrectomy involves removing approximately 75-80% of the stomach, resulting in a tube-like or “sleeve” structure. This significantly reduces the stomach’s capacity and helps in weight loss. Patients also benefit as the removed portion of the stomach is in part responsible for the production of hormones that drive hunger and cravings.

Benefits

Patients undergoing sleeve gastrectomy typically lose 50-70% of their excess weight. It also retains the natural function of the stomach without altering the digestive tract significantly, which can be beneficial for long-term nutritional health. Many patients like its relative simplicity and the fact that there is less rearrangement of the gastrointestinal tract.

SADI-S (Single Anastomosis Duodeno-Ileal Bypass with Sleeve Gastrectomy)

What is SADI-S?

The SADI-S procedure combines aspects of sleeve gastrectomy and intestinal bypass. It involves performing a sleeve gastrectomy followed by bypassing a portion of the small intestine.

Benefits

The SADI-S procedure offers substantial weight loss, comparable to gastric bypass, and is particularly effective in improving or resolving type 2 diabetes. By combining two weight loss mechanisms—restricting food intake and altering nutrient absorption—this procedure provides significant metabolic benefits. Patients undergoing SADI-S can expect profound changes in their overall health and quality of life due to the comprehensive approach to weight loss and metabolic improvement.

Why Choose Bariatric Surgery in Ireland?

Opting for bariatric surgery in your home country offers numerous advantages:

Familiar Environment: Undergoing surgery in a comfortable and familiar setting can greatly enhance the recovery process. Being close to home means being surrounded by family and friends who can provide emotional support.

High Standards: Benefit from Ireland’s renowned healthcare standards and professional expertise. Irish medical facilities are known for their high quality of care and advanced medical technology.

Consistent Aftercare: Access continuous, high-quality aftercare and follow-up services without the need for international travel. Ongoing support is crucial for long-term success, and having your healthcare team nearby ensures you receive the best post-operative care.

Cost-Effective: Avoid the additional costs and complications associated with travelling abroad for surgery. Staying in Ireland eliminates travel expenses and potential communication barriers, making the entire process smoother and more economical.

At Blackrock WeightCare, we are dedicated to providing first class bariatric surgery options tailored to your specific needs.
